In 2009, we have changed our application. To fill out an application, please download and print the PDF form below and bring it with you on your first visit. Applications may also be filled out at the office, but to save us a lot of time, we ask that you fill it out prior to arrival. Please also be sure to have your Drivers Lisence with you. We cannot complete the application process unless you do. Feel free to please give us a call or send us an email if you have any questions.
Requirements for Assistance
- Pets must be given fresh water everyday.
- All animals MUST be spayed and neutured. If they are not, they muct be within 3 months of enrollment in the program.
- All Animals must be caught up on shots. If they are not, they have 3 months to get all required shots and vaccinations.
- Attendance is required at at least 1 Healthy Pet class within the first 3 months.
- All animals must recieve medical attention in a timely manner if needed.
- Monthly check-ins via email, phone or message board are required so we know how your pets are doing.
- A minimum of 5 hours of time must be spent volounteering either at Daffy’s or another charity. A letter and contact information from the charity will be required.
- If at any time you can no longer care for your pet, you MUST let us know.
- You must be willing to let someone at Daffy’s visit with your pets to check on their condition should we feel the need to.
- A photo of you and your pets must eithe rbe brought with you, or sent via email.
- Your Drivers License or ID must be with you at every visit for identification verification purposes.
- Food Pick-up is once a month only.
- All food we give out is for use of the pets listed with us ONLY.
- Food assistance is for personal pets only. Any pets used for breeding, or any illegal activities are not eligable to receive assistance.
